Share your Prenuvo story

By checking the checkbox on the “Share your Prenuvo Story” webpage, you consent, agree and acknowledge that, in filling out the form on said website and responding to follow up communications from Prenuvo, Inc. (“Prenuvo”), you may be providing Prenuvo with images, graphics, photos, audio and video clips, written or other content, which may include your name, voice, likeness, image, and other biographical or related material (the “Materials”). You agree and acknowledge that the Materials may be used by Prenuvo and its licensees and assignees to promote or advertise Prenuvo and its products and services in all media throughout the world and in perpetuity, and hereby grant Prenuvo a perpetual, non-exclusive, fully paid and royalty-free, transferable, sub-licensable, worldwide license to use, edit, alter, copy, exhibit, create derivative works, publish or distribute the Materials, in whole or in part, throughout the world. You hereby waive any right of inspection or approval of the Materials or the uses to which such Materials may be put, and agree that Prenuvo will own any derivative works created from the Materials. You acknowledge that Prenuvo will rely on the licenses and permissions granted herein potentially at substantial cost to Prenuvo and hereby agree not to assert a claim of any nature whatsoever against anyone relating to the exercise of the licenses and permissions granted hereunder. You hereby represent and warrant that (i) you have full right and authority to grant the licenses and permissions herein, (ii) you are over eighteen (18) years of age, (ii) the Materials are true and accurate, (iii)no other party's authorization or consent is required with respect to the permissions and licenses granted to Prenuvo herein, (iv) you own the Materials or otherwise have the right to grant the rights and licenses set forth herein, and (v) the Materials do not violate, misappropriate or infringe on the rights of any third party, including, without limitation, privacy rights, publicity rights, copyrights, trademark and/or other intellectual property rights. By checking the checkbox on the “Share my Prenuvo Story” webpage, you agree to be contacted by Prenuvo regarding the Materials and agree that any follow up information you provide in such communications shall also be considered Materials.
